MEDICAL FORMS: DO NOT mail or fax forms back to camp.  All forms must be presented at camp check-in. 

ENROLLMENT: One month prior to camp a confirmation packet will be emailed. It will include a schedule of events, directions to Wake Forest University, arrival and departure times, a list of what to bring and a medical form. All medical forms must be completed before camp begins. No one will be permitted to participate without a completed medical form.

KEY DEPOSIT: For Residential Campers a key deposit of $80.00 is required at check-in. The deposit must be in the form of a cash payment and will be returned upon check-out.

CAMP STORE: At each of Jay Vidovich’s Wake Forest Soccer Camps there will be a camp store. The camp store will provide each camper with the latest in Wake Forest gear at discounted prices.

REFUND POLICY: Full refunds minus a $50.00 processing fee are available until June 1, 2012 for all campers.

AIRPORT TRANSPORTATION: Transportation to and from the Piedmont Triad International Airport in Greensboro NC is available for a fee of $50 (roundtrip) and $25 (one-way). Also transportation to and from Raleigh/Durham International Airport & Charlotte Douglas Airport is $100 (roundtrip) and $50 (one-way).  Please email us at WakeForestSoccerCamp@gmail.com to make arrangements.
